Oil Fired Hot Air or Boiler Cleaning $225.00*
This annual efficiency check, tune up and cleaning of heating equipment includes:
• Vacuum clean equipment and flue pipe, including chimney base (where accessible), as necessary
• Inspect smoke pipe, seal and clean openings
• Inspect chimney base
• Inspect flue pipe and proper flue draft
• Inspect combustion chamber
• Clean and test electrical controls
• Inspect and set electrodes
• Inspect and adjust proper air-oil ratio
• Inspect fan & air intake
• Inspect belts, pulley and couplings
• Oil all motors on burner, fans and circulators
• Inspect and adjust draft regulator
• Replacement of the fuel nozzle and in-line oil filter
• Replacement or cleaning of pump strainer
• Inspect exposed visible fuel oil tank and oil lines
• Check visible fuel oil level
• Install customer supplied Air Filter

Hot Water Heater Cleaning & Contract Oil Fired, Gas or Electric $150.00 **
Hot Water Heater coverage includes cleaning plus the repair or replacement of the following:

• Burner motor
• Cads Cell Eye
• Burner fan & coupling
• Fuel Oil Filter*
• Transformer
• Fuel nozzle*
• Cads cell relay
• Fuel nozzle assembly*

** Any service needed beyond standard cleaning will be billed at time and material.
* Will only be replaced at time of service if applicable.

Air Conditioning Tune-Up $150.00
Tune-Up and Efficiency check of AC equipment includes:

• Inspect and Clean Condensing Unit
• Flush drain line and condensate pump (if applicable)
• Inspect Evaporator Coil
• Inspect and tighten all electrical connections
• Inspect Fan, Motor & Belt
• Inspect System for Leaks
• Check Freon level
• Check Refrigerant Charge & Pressure
• Inspect general operation of unit

** Cleaning of the evaporator coil is additional if needed. The addition of Freon is an additional charge per pound, if needed. Any service needed beyond standard cleaning

HEATING & A/C EQUIPMENT MAINTENANCE AND AGREEMENT(S)

I have read & understand all the General Terms & Conditions below.
I agree & understand that in order for the heating equipment service agreement(s) to be valid; all home heating oil/fuel must be purchased exclusively from American Energy Supply.
I understand that my Heating Equipment must have an in-line fuel oil filter for Contract(s) to be effective and valid.

GENERAL TERMS & CONDITIONS

1. This contract is offered by American Service Dept. (will be referred to as “ASD” in these general terms & conditions); for customers who solely purchase home heating fuel from American Energy Supply (will be referred to as “AES” in these general terms & conditions).
2. This agreement is offered to all customers subject to inspection & approval of heating equipment. Upon inspection, if the heating equipment is found to be in unacceptable, poor or nonfunctional condition, any & all repairs to place heating equipment in contract condition will be charged at time & material rate. In order for this service agreement to be effective & valid, heating equipment must have an oil filter complete within the fuel oil line. If heating equipment does not have oil filter complete it will be billed at time & material. Upon inspection of the heating equipment; if any part(s) are found to be obsolete, it will be billable at Plan C cleaning rate. “ASD” is not responsible for replacing obsolete parts or any parts unavailable through normal local sources of supply. This service agreement refers to most domestic heating equipment & shall be limited to heating equipment rated at a maximum firing rate of 1.00 gallons per hour, any nozzle over will be an additional charge starting at $50.00 (Does not apply if heating equipment contract specifically states Commercial on reverse side).
3. This is a heating equipment service agreement & does not obligate “ASD”, for repairs due to heating equipment failures: defects in radiators, baseboard, registers, duct work, smoke pipe, plenum, sheet metal, water logged expansion tanks, water leaks, piping systems, flow valves, relief valves, pressure reducing valves, automation flue dampers, control sensing outside temperature, circulator bearing assemblies, zone valves, domestic hot water equipment & accessories, hot water coils, air conditioning equipment, electric switches, or failure of electricity.
4. This heating service agreement does not cover general electric, low pressure or vaporizing heating equipment, or steam heating equipment. This heating service agreement does not cover repair of service of heating equipment due to fire, flood, lightning, freeze-up, & any & all other Acts of God and/or Nature. This heating service agreement does not cover service calls caused by improper setting of the thermostat, blown fuses, breaker in the “off” position, any other switches in the “off” position (emergency switch, etc.), inadequate water in the boiler, tampering with equipment, or inadequate fuel in the fuel tank.
5. Under this service agreement it is not expressed or implied that “ASD” is responsible to fix, maintain or repair customers’ fuel tank(s), fill pipe(s), fuel line(s), chimney, or chimney liner. “ASD” is not liable for old, rusting, leaky, unleveled, leaning, or unsteady oil tank(s). Not liable for water, sludge, or any other foreign debris in oil tank(s) or oil line(s). “ASD” shall not be liable for any loss, damage, or injury which results directly or indirectly from the condition of the customers’ fuel tank(s), fuel line(s), fill pipe(s), chimney, or chimney liner. This service agreement is not an agreement to deliver or automatically deliver fuel oil by American Energy Supply. Customer is responsible for arranging any & all fuel deliveries from American Energy Supply. If heating equipment fails due to little or no oil in customers’ fuel tank(s), customer is solely responsible & will be billed at time & material rate. If customer runs out of oil & the heating unit needs to be primed & started, customer is solely responsible & will be billed at time & material rate.
6. This agreement is available as an additional service to buyers of American Energy Supply’s fuel oil only. This service agreement will automatically terminate if the customer ceases to use “AES”’s fuel oil. If a customer purchases fuel oil from any other fuel company beside “AES”, the contract is VOID. Fuel oil must be purchased on a regularly (monthly) basis, if a customer is not completely filling tank at time of delivery, within the service contract year. This service agreement will automatically terminate if the customer permits anyone other than “ASD”s’ employees or agents to perform service to the heating equipment. There shall be no obligation by “ASD” to perform any service or provide parts after termination of this service agreement for any of the above stated reasons.
7. American Service Dept. is not responsible for furnishing a new hot air unit, gas unit, hot water heater or boiler at any time under this service agreement. 20% OFF All billable service; Parts and Labor does not apply to new heating or a/c equipment.
8. This agreement is non-transferable. Non-transferable if customer moves to new home or apartment. Nor is the contract valid if a customer sells property to new owner.
9. Failure to maintain a satisfactory credit status with “AES” or “ASD” automatically terminates this service agreement without a refund. This service agreement will automatically terminate if the customer fails to pay for all fuel oil, parts, labor or any other chargeable item when due. On balances of 25 days or more, we will compute a finance charge at 1.8334% per month. You are also subject to a $36.00 or more charge for any Non-sufficient Funds or Stop Payment on all checks. You will be in default if you do not pay balance within 30 days from the time the service work is performed. You are responsible for any & all billable service work performed on heating equipment not covered under the service contract or general conditions. Default means “ASD” can demand immediate payment of the full balance, which includes all finance charges & any other collection charge. If we refer collection of the balance owed to an attorney or collection agency, you will be liable for all reasonable collection or attorney’s fees incurred plus all court costs & expenses.
10. Under no circumstances shall “ASD” be liable for any consequential or resultant damages or injury, including death, to persons or property, nor from damages resulting from defective heating equipment, or for loss or damage resulting from operation or non-operation of said heating equipment. All promises & understanding are contained in this service agreement. No additional promises on the part of “ASD” or modifications to this agreement shall be binding unless made in writing & executed by an authorized representative of “ASD”. Delays in performing services, making repairs, installation of parts due to strikes, accidents, explosions, labor shortages, or other conditions beyond the control of “ASD” shall excuse the performances of this service agreement.

Agreement scheduled after 30 October of the current year will be valid until 15 June of the following year. We reserve the right to inspect equipment prior to maintenance agreement coverage. If your heating equipment fails inspection you will be billed at time and material. If you have prepaid for a contract your payment will go toward the cost of the cleaning at time and material. 24 hour service begins October 15th & ends March 31st, “ASD” will service & repair heating unit within 24 Hours of time the service call is placed, when average temperature is below 32 degrees fahrenheit. “ASD” reserves the right to repair the service contract call during normal business hours; between the hours of 08:00 and 16:00. Any request for repair after normal business hours will be charged at contract labor rate. “ASD” requests a 24 hour cancellation notice for all scheduled heating equipment cleanings. Under the Unfair Trade Practices and Consumer Protection Law, you, the buyer, may cancel this transaction at any time prior to midnight of the third business day after the date of this transaction. See the attached notice of cancellation form for an explanation of this right. If you, the buyer opt to cancel this service agreement you, the buyer, will be billed at time for the services of cleaning the heating unit. By accepting the cleaning of the heating equipment customer accepts general terms & conditions of this Service Agreement. Violation of any part of the general conditions will terminate this service agreement, & there shall be no refund or credit allowed.
